You’ll facilitate timely referrals to palliative care, mental health services, guardianship or conservatorship processes when appropriate, and community-based services that prevent readmission. You’ll document findings meticulously, maintain compliance with regulatory standards, and ensure family-centered communication that respects cultural values and language needs.
