We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN / T-LPN) to join our team in the Mineral Springs area of North Conway, NH. This role focuses on providing high-quality patient care during the overnight shift and supporting a positive, safe, and patient-centered environment. The ideal candidate will be reliable, detail-oriented, and able to work independently while collaborating closely with the clinical team.
Provide direct nursing care to patients under the supervision of an RN or clinical leadership
Monitor, document, and communicate changes in patient condition
Administer medications and treatments in accordance with state regulations
Perform wound care, injections, and routine nursing procedures
Assist patients with activities of daily living (ADLs) as needed
Support care teams during assessments, procedures, and emergencies
Maintain accurate and timely documentation in medical records
Promote infection control and safety standards throughout the shift
Communicate effectively with patients, families, and interdisciplinary staff
Active New Hampshire LPN license (or compact-eligible)
BLS/CPR certification (current or able to obtain before start date)
Strong clinical judgment and assessment skills
Ability to work effectively during overnight hours
Previous experience in long-term care, assisted living, rehabilitation, or similar settings preferred
Reliable, professional, and patient-focused
