Job Description

Ultimate Care NY is seeking a Home Care Registered Nurse to join our team on a per-diem basis. As a Home Care Registered Nurse, you will provide care to patients in their homes and help them manage their medical conditions. Your responsibilities will include assessing patient needs, developing care plans, administering medication, and managing medical equipment. With your support, we aim to ensure that patients receive the highest quality of care and feel comfortable and confident in their homes. Ultimate Care has been providing home health care services that meet the tailored needs of thousands of New Yorkers for the past 20 years. Corporate Office Location:

New City, NY (Rockland County)

Middletown, NY (Orange County)

Responsibilities

• Conducting patient assessments and developing individualized care plans.
• Administering medications and treatments as prescribed by physicians.
• Educating patients and their families about their medical conditions and care plans.
• Monitoring patient progress and adjusting care plans as needed.
• Coordinating with other healthcare providers to ensure comprehensive care.
• Documenting patient care and maintaining accurate medical records.
• Responding to patient calls and inquiries in a timely manner.

Requirements

• Active New York State Registered Nurse license.
• At least 1 year of experience in home health care or a related field.
• Knowledge of state and federal regulations related to health care services.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and respect for privacy.
• Reliable transportation and willingness to travel to patients' homes.
• CPR certification.

Compensation: $90,000-95,000 annually + $5,000 Sign-On Bonus + opportunity for additional per diem rate of $85-95 on top of salary for additional visits. 

Per Diem:

Per Diem: $85-95 per assessment.

Start of Care: $90-$95 per visit

Revisit: $80-$85 per visit
