Starting Salary Range: $59,773-62,000 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change
The Central Access Intake Coordinator provides professional, timely and welcoming responses to individuals seeking services for themselves or someone they know or provide services to. The goal of the team is to improve system efficiency by simplifying methods to access existing community-based services targeted to individuals who frequently utilize community-based and state operated hospital systems. The program will provide a central point of contact referrals and initial coordination of services for individuals, community stakeholders, and referring providers. The Intake Coordinator will be responsible for assessing the needs of callers, providing education on service options available, facilitating completion of steps needed for referral and intake into specific programs. The Central Access Team is currently in development and as such the role will evolve based on the needs of both internal and external stakeholders. This position is funded through a State of Minnesota Mental Health Innovations Grant.
Duties & Responsibilities:
Serve as a first point of contact for individuals seeking services and community providers looking to make referrals.
Assess needs, provide education, and make recommendations on RADIAS Health service options that the individual may to eligible to receive.
Facilitate gathering of records and assessment information needed to determine eligibility for specific programs. Provide updates to the individual/referral source on next steps, estimated timelines, and other updates related to service eligibility.
Partner with internal programs and facilitate work completion needed to maximize timely response to referents and associated admission into care.
Partner with external stakeholders to provide education, information and streamlining of referral pathways, with an emphasis on welcoming diverse and underserved communities into the RADIAS system of care.
Provide education and assistance connecting individuals to needed community resources.
Develop and maintain systems for tracking service needs in relation to program capacity, which may include maintenance and follow up with wait lists.
Participate in the management of service availability through partner systems such as MN Fast-Tracker.
Participate in the gathering and management of data needed for State outcomes reporting.
Provide initial cultural assessment to guide how the agency may best respond to the individual needs of each person seeking services.
Complete crisis assessments, functional assessments and provide short-term services as needed, while connecting the individual to on-going care providers.
Ensure appropriate reporting is submitted to Vulnerable Adult, Child Protection, the Ombudsman’s Office, or other agencies/entities when necessary.
Provide courteous and respectful services to all individuals, community members, external and internal partners.
Effectively communicate, track and coordinate all aspects of the process necessary to facilitate enrollment in RADIAS Health services.
Maintain knowledge of community resources, Medicaid benefits and information on accessing community social services and health insurance benefits.
Maintain a pattern of regular work hours.
Must be able to transport clients in the community.
Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
Other duties as assigned.
Bachelor's degree in behavioral science or related fields; and
2,000 hours of supervised experience in the delivery of services to persons with a mental illness.
