Case Management Registered Nurse (RN) - Travel Position Location: Vicksburg, MS Job Type: Travel Contract Duration: 13 weeks Shift: 5x8s Days Pay: $2108/week Completion Bonus: Up to $7,500 total. (You will receive $2,500 after the successful completion of each of your first 3 contracts with us) Job Description: We are seeking a highly organized and compassionate Case Management Registered Nurse (RN) to coordinate care for patients across the healthcare continuum. This role is vital in ensuring patients receive the right care at the right time, while also facilitating a smooth transition from one level of care to another. The Case Management RN will act as a patient advocate, working to optimize clinical outcomes, manage healthcare resources, and prevent hospital readmissions. The ideal candidate is an excellent communicator, a proactive problem-solver, and has a strong understanding of discharge planning and community resources. Key Responsibilities:

• Conduct comprehensive assessments of patients' medical and psychosocial needs.
• Develop, implement, and monitor individualized care plans in collaboration with the healthcare team, patients, and families.
• Coordinate with physicians, therapists, and social workers to facilitate a smooth and timely patient discharge.
• Educate patients and families on their condition, treatment plan, and available resources.
• Identify and address potential barriers to patient care and recovery.
• Ensure efficient use of healthcare resources and manage patient-related costs.
• Maintain accurate and detailed documentation of all case management activities.

Qualifications:

• Active RN license in Mississippi.
• Associate’s or Bachelor’s Degree in Nursing.
• Basic Life Support (BLS) certification.
• Minimum of 1-2 years of recent clinical nursing experience.
• Prior experience in case management or utilization review is highly preferred.
• COVID vaccine and recent TB test may be required.
